In addition to the General Terms & Conditions applicable to all our conferences, the following specific Terms & Conditions apply to the October 2008 International Mediterranean Conference aboard the Azamara Quest:

1. Full registration fee plus accommodation deposit required at time of initial registration to complete registration and secure cabin. Deposit amount depends on cabin type: $500 US per person for inner; $600 US per person for oceanview; $700 US per person for balcony. Travel/cruise insurance is recommended.

2. Balance of final payment due July 19, 2008.

3. Cancellation penalties are charged by the cruiseline for cabin cancellations less than 70 days prior to the cruise sailing date, as follows: 69-30 days prior, deposit amount; 29-15 days prior, 50% of cabin amount; 14 days or less, no refund.

4. Accommodation bookings for the cabins held by the Course must be done through Marlin Travel (Sherway Gardens, Toronto). If making air travel arrangements independently, please ensure sufficient time to reach the port prior to cruiseship departure.

5. In the event a registrant is unable to attend the conference, substitution of registrants is permissible without any additional registration charge for attending the conference and receiving conference materials. However, registrant is responsible for any additional travel or accommodation change charges or penalties which may be levied by the airline, cruiseline, or other agency.

6. For refund of registration fees, if a registrant is unable to attend and unable to find a substitute, notification of cancellation should be sent to the Psychiatry Review Course in writing by mail, fax, or e-mail. For cancellations received prior to September 11, 2008, registrants have the option of (i) receiving a credit voucher for the full registration amount paid, redeemable towards any future Psychiatry Review Course or MD Conferences meeting; or (ii) receiving a refund of registration fees paid less a $150 administrative charge. For cancellations received September 11, 2008, or later, no credit or refund will be issued.
