In addition to the General Terms & Conditions applicable to all our conferences, the following specific Terms & Conditions apply to the October 2006 International Conference aboard the Celebrity Millennium:

1. Full registration fee plus accommodation deposit of $1,250 (US) per cabin required at time of initial registration to complete registration and secure cabin. Travel/cruise insurance is recommended.

2. Balance of final payment due July 19, 2006.

3. To register for conference, accommodation bookings for the cabins held by the Course must be done through Thomas Cook Travel (Sherway Gardens, Toronto). Air travel arrangements may be made independently, or using "airmile" points (which Thomas Cook can also assist in booking). Please ensure sufficient time to reach the port for departure.

4. For cancellations prior to or on June 5, 2006, $350 US per cabin of initial accommodation deposit non-refundable; for cancellations after June 5, 2005, and before July 19, 2006, $900 US per cabin of initial accommodation deposit non-refundable. Accommodation deposit and accommodation charges fully non-refundable for cancellations July 19, 2006 and later.

5. In the event a registrant is unable to attend the conference, substitution of registrants is permissible without any additional registration charge for attending the conference and receiving conference materials. However, registrant is responsible for any additional travel or accommodation change charges or penalties which may be levied by the airline, cruiseline, or other agency.

6. For refund of registration fees, if a registrant is unable to attend and unable to find a substitute, notification of cancellation should be sent to the Psychiatry Review Course in writing by mail, fax, or e-mail. For cancellations received prior to September 1, 2006, registrants have the option of (i) receiving a credit voucher for the full registration amount paid, redeemable towards any future Psychiatry Review Course; or (ii) receiving a refund of registration fees paid less a $75 administrative charge. For cancellations received September 1, 2006, or later, no credit or refund will be issued.
